Maintain dignity and quality of life when your journey in this life is nearing its end.
Quality of life is vital at any stage, but never more important when you have been given a terminal diagnosis. You want to live out the remainder of your days on your terms. You seek peace and no longer desire treatment measures. PSL can help you and your family during this time. Dignity is promoted by minimizing pain, discomfort, and providing active support of the psychological, social, emotional and spiritual needs of you and your family throughout the continuum of care, including during the dying process.
PSL works with families and residents to create individualized care plans when the end of life is imminent. The goal is to honor the remainder of your time on this earth. We work with you to minimize pain and symptoms and improve comfort not only physically but emotionally, socially, intellectually and spiritually for you and your support system. At times, this care may be supplemented with hospice services from an outside agency cooperatively supporting the individual with the PSL team and family.

It is always advised to have a living will or advance directive to determine and tell others what care you desire, even if you are too weak or ill to verbalize these desires. A living will or advance directive is a document that shares what support and care you would want in certain situations. It can ease the burden on family and friends and ensure you have your wishes met at the end of life. PREPARE For Your Care, funded in part by the National Institute on Aging, is an interactive online program that helps you fill out an advance directive and express your wishes in writing. This tool is available in English and Spanish.
Costs vary and are determined based on your insurance provider.
